I'm using a X7DWN+ main board with an 3Ware 9650SE-16ML controller (Firmware: 4.06.00.004 Boot-Loader: 3.08.00.001 Bios: 4.05.00.015) and 16 1TB Seagate ST31000340NS drives. We have multiple systems and all show the same behavior. The systems randomly loses hard drives in the ARRAY every few days. The drives are not bad and will rebuild the ARRAY, but the drives fall off line. What could be the issue?

We have seen this issue before, after upgrading the firmware of the 3Ware 9650SE-16ML to revision 3.08.00.016 this issue is resolved.
