We have your AOC-USASLP-H8IR card, and the driver has detected a device with old or out-of-date firmware. The device will not be used. Reboot drives did come back but we are worried about the error and wonder if it will happen again. This is a large 4TB SAN Storage server.

Check the firmware code on the HDD. Also if HDD manufacturer provided a debug tool, download it and run to see if tool can get any debugging result or useful indication.
