Question
This article in http://www.supermicro.com/support/faqs/faq.cfm?faq=8467 is also related to the following issue:

One Linux CentOS 5 machine (with latest kernel updates) with X7DBU & SIMSO+ (firmware 1.59 – 1.63) has the following problem: No KVM keyboard and mouse is working after Linux Boot, with the following errors detected:

hub 1-0:1.0: 6 ports detected
usb 1-6: new high speed USB device using ehci_hcd and address 2
usb 1-6: unable to read config index 0 descriptor/start
usb 1-6: can't read configurations, error -71

After flashing the FW: 1.49.02 Firmware from the above FAQ Article, the KVM is working again within Linux:

[root@vserv73 ~]# dmesg
usb 4-2: new full speed USB device using uhci_hcd and address 2
usb 4-2: not running at top speed; connect to a high speed hub
usb 4-2: configuration #1 chosen from 1 choice
input: Peppercon AG Multidevice as /class/input/input4
input: USB HID v1.01 Mouse [Peppercon AG Multidevice] on usb-0000:00:1d.2-2
input: Peppercon AG Multidevice as /class/input/input5
input: USB HID v1.01 Keyboard [Peppercon AG Multidevice] on usb-0000:00:1d.2-2

Can you fix this in firmware 1.63, too?

Answer
The firmware 1.49 in our FAQ is a modified firmware that actually supports USB 1.1 only. That's why it's working in your CentOS 5 kernel. The OS you are using doesn’t seem to support USB 2.0 correctly. Please download the firmware 1.63 with USB1.1 support below to flash your IPMI firmware:

SIMxx IPMI firmware 1.63 with USB 1.1: ugsim163-USB1-1.bin

You can also use a newer OS like CentOS 5.4. We had tested it without any similar issue.
